Zusammenfassung
Ein Aufenthalt in mittleren Höhen und darüber bedeutet für das Herz-Kreislaufsystem des Menschen eine gravierende Umstellung im Vergleich zu den Lebensbedingungen in einer Seehöhe < 2000 m. Durch Akklimatisationsvorgänge stellt sich der Kreislauf nach einigen Tagen auf den niedrigeren Luftdruck und den damit einhergehenden niedrigeren Sauerstoffpartialdruck in größerer Höhe ein, wobei diese Anpassungsvorgänge mit einem deutlich stimulierten Sympathikotonus einhergehen. Blutdruckerhöhend wirken zusätzlich die in der Höhe zu beobachtende Verminderung des Gesamtkörperwassers sowie die Erhöhung des Hämatokrit, der initial durch die Dehydratation und in weiterer Folge durch eine Stimulation der Erythropoese zustande kommt. Patienten mit arterieller Hypertonie sollten daher vor Antritt eines Aufenthaltes in Höhen > 2500 m kardiovaskulär und pulmonologisch auf ihre Tauglichkeit dafür getestet werden. Ein ideales Untersuchungsinstrument hierfür stellt die Ergometrie dar. In diesem Kapitel werden wichtige Empfehlungen für Hypertoniker und Patienten mit koronaren Herzkrankheiten gegeben.
