Zusammenfassung
„Errare humanum est“, jeder Mensch macht unweigerlich Fehler, niemand macht Fehler prinzipiell absichtlich, und nahezu jeder Fehler hat eine systemische Komponente.
Viele Studien haben gezeigt, dass Fehler gerade in dem komplexen System Krankenhaus ein relevantes Problem darstellen, und dort v. a. im Akutbereich. Eine Vielzahl von Faktoren und das Versagen oder Fehlen von Schutzmechanismen führen zum Endpunkt medizinischer Behandlungsfehler, der in der Statistik der Todesursachen an prominenter Stelle rangiert.
Eine Zentrale Notaufnahme (ZNA) als Drehscheibe in einem Akutspital ist ein Hochrisikobereich; die hohe Komplexität durch Zeitdruck, Unplanbarkeit, Interdisziplinarität und Multiprofessionalität mit zahlreichen Schnittstellen bietet mannigfaltige Fehlerquellen.
Eine wesentliche Schlüsselerkenntnis ist, dass sich die Arbeitsumgebung v. a. an den Akteuren orientieren muss. Es gilt, individuelle medizinische Dienstleistungen als hochqualitative Kernprozesse strukturiert abzubilden und ein optimales, auf den Menschen mit seinen Stärken und Schwächen abgestimmtes Umfeld zu etablieren, um Spitzenleistungen verantwortungsvoll und fehlerfrei erbringen zu können.
