Zusammenfassung
Die Königsdisziplin der Fortbewegung ist viel untersucht und doch immer wieder ein Rätsel. In der Literatur finden sich zahlreiche klinische Bilder und noch mehr Interpretationen der biomechanischen Vorgänge, die zu diesen Abweichungen führen. Aber nicht jede Theorie ist begründet und es gibt keine Universallösung. Um den Einfluss von Spastik bzw. Plus-Symptomatik und/oder Schwäche oder von strukturellen Bewegungseinschränkungen zu erkennen, ist es unabdingbar, das „normale Gehen“ zu verstehen. Standardparameter wie Kadenz, Schrittlänge, Gangdeviation, Armpendel etc. sind da nur mäßig geeignet. Es empfiehlt sich, das Gehen in seine Phasen zu unterteilen und Phase für Phase das Gangbild zu analysieren und Hypothesen aufzustellen. Diese Hypothesen müssen dann durch klinische Tests wie Muskellängen, Muskelkraft, Tonus, Tiefensensibilität etc. bestätigt oder widerlegt werden. Nur dann ist eine optimale physiotherapeutische Behandlung oder Orthesenversorgung möglich. Dieses Kapitel soll einen Einblick in die komplexe Behandlung von Gangstörungen bei neuroorthopädischen Krankheitsbildern liefern. Die Unterscheidung in spastisch, schlaff und peripher ist schwierig, weil unabhängig von der Diagnose ähnliche Abweichungen möglich sind. Allerdings gibt es innerhalb der Subgruppen bestimmte Häufungen, die auf den folgenden Seiten näher beleuchtet werden sollen.
