Zusammenfassung
Die infantile neuroaxonale Dystrophie (INAD) und das Cockayne-Syndrom (CS) sind zwei seltene autosomal-rezessiv vererbte neurodegenerative Erkrankungen, die bei typischem Verlauf in den ersten beiden Lebensjahren beginnen und einen schweren, im 1. bis 2. Jahrzehnt zum Tode führenden Verlauf zeigen. Ätiologisch und pathophysiologisch haben sie nichts gemeinsam. Mutationen im PLA2G6-Gen bei INAD führen zu mitochondrialen Funktionsstörungen und Eisenakkumulation im Gehirn, während bei CS Mutationen der Gene ERCC6 oder ERCC8 zu gestörten DNA-Reparaturmechanismen und vorzeitigen Alterungsprozessen beitragen. Das Kapitel beschreibt die Wege zur Diagnose, eine spezifische Therapie ist nicht verfügbar.
