Zusammenfassung
Der Verlauf bei dieser jungen Patientin mit initial 4-Fragment-Humeruskopffraktur ist nicht unerwartet. Bis heute wird kontrovers diskutiert, ob primär eine Schultertotalprothese auch bei jungen Patienten implantiert werden soll. Dies in Kenntnis der unter Umständen kürzeren Standzeiten der Prothese – wie an diesem Beispiel auch klar ersichtlich ist. Für uns stellte sich die Frage bei dokumentierter distaler Perforation der Prothesenspitze, ob zugewartet werden kann, wie der Patientin andernorts vorgeschlagen oder in Kenntnis der schwierigen Revision aktiv vorgegangen werden soll. Wir haben uns für die zweite Option entschieden.
