Zusammenfassung
In Deutschland werden pro Jahr ca. 3000 Organe nach postmortalen Organspenden übertragen. Grundsätzlich ist jeder Hirntote ein potenzieller Organspender. Alle deutschen Kliniken sind zur Meldung potenzieller Organspender und zur Mitarbeit am Organspendeprozess verpflichtet. Die rechtlichen Grundlagen regelt das bundesdeutsche Transplantationsgesetz. Eine der unabdingbaren Voraussetzungen für die Organspende ist die Feststellung des Hirntods nach den Richtlinien der Bundesärztekammer. Der Hirntod ist mit komplexen pathophysiologischen Veränderungen der kardiopulmonalen Funktion sowie der Flüssigkeits, Elektrolyt- und metabolischen Homöostase verbunden. Mit der Feststellung des Hirntods beginnt bei entsprechender Organspendeerklärung des Verstorbenen die zielgerichtete organprotektive Therapie, die letztendlich auf die optimale Organperfusion und -oxygenierung fokussiert. Die Qualität dieser Organprotektion hat direkten Einfluss auf den Transplantationserfolg.
