Zusammenfassung
Die Anlage eines Tracheostomas ist ein seit der Antike anerkanntes und etabliertes Verfahren zur Verhinderung des Erstickungstodes. In den folgenden Jahrhunderten wurde die Technik der Tracheostomaanlage immer weiter verfeinert und ist auch noch heute fester Bestandteil der chirurgischen Tätigkeit. Mit der Entwicklung der Intensivmedizin hat sich das Indikationsspektrum zur Tracheotomie erweitert, allerdings wurden auch neue, nichtoperative Verfahren eingeführt, welche mit der Technik der klassischen Tracheostomaanlage konkurrieren und diese teilweise ersetzen. Neben der noch immer durchgeführten Notfallanlage bei akuter respiratorischer Insuffizienz hat sich die plastische Tracheotomie als elektive Operation bei Langzeitbeatmung sowie – durch eine Reduktion des Totraumes und eine Minderung des Atemwegswiderstandes – im Rahmen des Weanings etabliert.
