Zusammenfassung
Über 90 % aller soliden Pankreastumoren sind duktale Adenokarzinome. Typische Erstsymptome sind persistierende, dumpfe Oberbauchschmerzen mit Ausstrahlung in den Rücken und bei Karzinomen im Pankreaskopf auch ein Verschlussikterus durch Kompression des Gallengangs. Spezifische laborchemische Tests fehlen. Tumormarker (insbesondere CA19-9) nützen vor allem zur Verlaufsbeurteilung bei anderweitig gesichertem Tumor, da auch bei benignen cholestatischen Erkrankungen deutlich erhöhte Werte beobachtet werden. Wesentliche bildgebende Verfahren sind die Abdomensonographie zum Screening bzw. zum Ausschluss von Begleiterkrankungen und die kontrastverstärkte Computertomographie in hochauflösender Dünnschichttechnik zur Beurteilung der Operabilität. Die Endosonographie eignet sich wegen ihrer hohen Ortsauflösung vor allem zur Früherkennung kleiner Tumoren und zur differenzialdiagnostischen Klärung z. B. bei zystischen Läsionen mit der Möglichkeit zur gezielten Punktion. Als Alternativerfahren und zur Verlaufsbeurteilung ist die MRT vor allem bei zystischen Läsionen hilfreich.
