Zusammenfassung
An den Folgen hypertensiver Schwangerschaftserkrankungen wie der Präeklampsie, Eklampsie und dem HELLP-Syndrom („hemolysis, elevated liver enzymes, low platelet count“) sterben weltweit jährlich mehr als 300.000 Schwangere. Die einzige kausale Therapie ist die Beendigung der Schwangerschaft. Auch peripartale Blutungskomplikationen sind sowohl in Ländern mit hohen Ausgaben für das Gesundheitssystem und umso mehr in ärmeren Ländern nach wie vor relevante Ursachen für mütterliche peripartale Todesfälle. Andere, gleichwohl seltenere, Ursachen für bedrohliche Akutsituation in der Schwangerschaft entstehen im Gefolge von Fruchtwasserembolien sowie dem Auftreten einer peripartalen Kardiomyopathie. Darüber hinaus stellt die interdisziplinäre und interprofessionelle Zusammenarbeit bei jeglicher Art schwangerschaftsassoziierter Notfälle eine Herausforderung dar und sollte Anlass geben, Schnittstellen ins Visier zu nehmen und Kommunikationsabläufe zu prüfen und zu hinterfragen.
Dieses Kapitel stellt die Pathophysiologie ausgewählter schwangerschaftsassoziierter Notfälle und deren intensivmedizinisches sowie geburtshilflich-anästhesiologisches Management dar.
