Zusammenfassung
Die Beurteilung der kindlichen Entwicklung ist eine zentrale Aufgabe des Kinder- und Jugendarztes. Sie ist immer Teil der 1971 eingeführten und 2006 erweiterten Früherkennungsuntersuchungen (U1–U11, J2). Oft fragen aber auch die Eltern nach der Entwicklung ihres Kindes, vor allem in den ersten Lebensjahren. In diesem Lebensabschnitt werden genetisch bedingte wie auch prä- und perinatal erworbene Entwicklungsstörungen überwiegend manifest. Die Abklärung der Ätiologie und die Einleitung einer adäquaten Therapie und Entwicklungsförderung stellen bei abweichender Entwicklung die weiteren Bausteine des Vorgehens dar. Eine medizinisch-kausale Therapie ist bisher allerdings nur bei einzelnen behandelbaren Krankheitsbildern möglich, wozu einige angeborene Stoffwechselkrankheiten zählen. Für die Mehrzahl der Entwicklungsstörungen stehen medizinisch orientierte (Physiotherapie, Ergotherapie, Logopädie) und psychologisch-pädagogische Fördermaßnahmen im Vordergrund. Diese sollten rational begründet und an die Symptomkonstellation angepasst eingesetzt werden. Hierzu ist heute der Bezugsrahmen der 2007 von der WHO verabschiedeten International Classification of Functioning, Disability and Health, Children and Youth (ICF-CY) nutzbar.
