Zusammenfassung
Gasbrand, als fulminant verlaufende, nekrotisierende bakterielle Infektion des Weichteilgewebes verursacht durch Clostridiumspezies, ist als eine seltene Komplikation nach Verbrennungsverletzung anzusehen. Daher kommt der Anamneseerhebung unter Kenntnis eines natürlichen Clostridiumreservoirs, perforierender Begleitverletzungen des Weichteilgewebes bei Verbrennungen sowie Vorerkrankungen, welche ein ischämisch, anaerobes Wundmilieu begünstigen, besondere Bedeutung zu. Eine evidenzbasierte Datenlage hinsichtlich einer effektiven Prophylaxe und Therapie beruht vornehmlich aus Erfahrungen und Fallstudien im militärischen, nichtzivilen Bereich. Frühzeitige Diagnosestellung und radikales chirurgisches Vorgehen in einem intensivmedizinischen Kontext sind entscheidend im Hinblick auf Überleben und Behandlungserfolg. Die hyperbare Sauerstofftherapie hat ihre Effektivität in Einzelfällen bewiesen, gilt jedoch bisher nicht als Standartbehandlung. Eine eventuelle Antibiotikaresistenzbildung wurde bisher nicht als Problem beschrieben.
