Zusammenfassung
Eine Herzinsuffizienz liegt vor, wenn das Herz nicht in der Lage ist, ein für den metabolischen Bedarf des Organismus ausreichendes Herzminutenvolumen zu fördern.
Im Kindesalter beruht Herzinsuffizienz meistens auf einer Volumen- und/oder Druckbelastung durch angeborene Herzfehler. Selten ist sie auf eine primäre Myokardinsuffizienz, d. h. ungenügende myokardiale Kontraktilität, z. B. infolge Reduktion der Zahl kontraktiler Elemente bei Kardiomyopathie, arterielle Hypertension, tachy- und bradykarde Rhythmusstörungen oder metabolische Störungen, zurückzuführen.
Bei unzureichendem Herzminutenvolumen werden verschiedene neurohumorale Regulationsvorgänge aktiviert, speziell das Renin-Angiotensin-Aldosteron-System und der Sympathikotonus. Die gesteigerte Aldosteronproduktion führt über eine vermehrte Rückresorption von Natrium, Chlorid und Wasser zur Erhöhung des intravasalen Volumens und des venösen Drucks im großen und kleinen Kreislauf. In einem frühen Stadium der Herzinsuffizienz ist dieser Anstieg der sog. Vorlast für die Verbesserung des Schlagvolumens nützlich, da über den Frank-Starling-Mechanismus die Kontraktionskraft des Herzens erhöht wird. Die Vergrößerung des diastolischen Ventrikelvolumens verursacht aber eine erhöhte Wandspannung mit Steigerung des myokardialen Sauerstoffverbrauchs. Bei stark erhöhter Vorlast kommt es schließlich zu Stauungssymptomen im Lungen- und/oder Körperkreislauf.
