Zusammenfassung
Eine Suppression des Immunsystems ist entweder durch eine Erkrankung oder durch ärztliche Intervention bedingt. Immunsupprimierte Patienten präsentieren sich nicht als homogene Gruppe, vielmehr wirken immunmodulierende Einflüsse im Rahmen verschiedener Erkrankungen und Therapien in ganz unterschiedlichem Maße auf die zahlreichen Effektoren und Mechanismen der Wirtsabwehr ein. So verkörpert beispielsweise die Neutropenie eine schwere Insuffizienz der angeborenen, unspezifischen Immunität, während eine Beeinträchtigung der zellulären bzw. humoralen Immunität (T- und B-Lymphozyten) die erworbene, spezifische Immunität betrifft. Meistens variiert das Ausmaß der Immunsuppression in Abhängigkeit vom Krankheitsverlauf, weshalb die zeitliche Entwicklung (Dauer) der Störung ein wesentliches Kriterium für die Schwere der Immunsuppression darstellt. Aus diesen Faktoren resultiert, dass immunsupprimierte Personen entweder generell oder auch während bestimmter Phasen ihrer Erkrankung für einzelne Erreger bzw. Erregergruppen besonders anfällig sein können. Das Ziel geeigneter Hygienemaßnahmen muss sein, das Infektionsrisiko dieser Patienten effektiv zu minimieren. Die Maßnahmen umfassen die Erarbeitung von angepassten Hygienestandards, die Umsetzung der Standards in die Praxis sowie die Surveillance relevanter Infektionen.
