Zusammenfassung
Das Prostatakarzinom ist weltweit beim Mann die zweithäufigste Krebserkrankung mit den höchsten Inzidenzraten in Australien und Neuseeland, Westeuropa und den Vereinigten Staaten. Die Prostatakarzinom-Sterblichkeit sinkt tendenziell in Ländern mit hohem Lebensstandard, während sie in solchen mit geringerem Wohlstand eher steigt. Die Ätiologie der Erkrankung ist noch weitgehend unbekannt, ein hohes Alter gilt als Hauptrisikofaktor. Zur Prävention des Prostatakarzinoms sind bisher keine Medikamente zugelassen. Durch Prostatakarzinomscreening kann die Sterblichkeit am Prostatakarzinom gesenkt werden, unklar ist jedoch, ob die damit verbundenen Überdiagnose und Übertherapie mit Auswirkungen auf die Lebensqualität in einem sinnvollen Verhältnis zum vermuteten Nutzen des Screenings stehen. Der natürliche Verlauf des Prostatakarzinoms ist meistens eher langsam progredient, bei gut oder mäßig differenzierten lokal begrenzten Tumoren treten selten Todesfälle innerhalb von 10 Jahren nach Therapie auf. Es besteht kein gesicherter Zusammenhang zwischen Alter und Biologie des Prostatakarzinoms.
