Zusammenfassung
Bei den komplexen Umständen und den dargestellten Bedürfnissen des diabetischen Fußsyndroms ergibt sich die Notwendigkeit der multidisziplinären und multiprofessionellen Teambetreuung. Wichtig ist die sektoren- und fachübergreifende ärztliche Kooperation ebenso wie die Integration der nicht-ärztlichen Assistenzberufe (Diabetesberater/innen, Podologen/innen und orthopädische Schuhmachermeister/innen). Gemeinsames Ziel aller Beteiligten muss, bei optimaler Koordination der zur Verfügung stehenden Ressourcen, eine hohe Abheilungsrate sein. Weiterhin ist auf einen ausreichend funktionalen (Rest-)Fuß und eine geringe Rezidivrate (Sekundärprävention; Einlagen- und Schuhversorgung) zu achten. Die erarbeiteten Strukturen der AG Fuß der Deutschen Diabetesgesellschaft und deren Zertifizierungsverfahren stellen hierfür ein erfolgreiches Model dar, das auch in der nationalen Versorgungleitlinie exemplarisch genannt wird.
