Zusammenfassung

Neuromodulationsverfahren haben die früher praktizierten Läsionsverfahren in der invasiven Schmerztherapie weitgehend abgelöst. Heute versucht man durch die Applikation elektrischer Reize oder durch die Gabe intraspinal verabreichter Medikamente eine Beeinflussung der Schmerzleitung und verarbeitung zu erzielen. Die Vorteile der Modulationsverfahren sind ihre Reversibilität, die Möglichkeit einer Austestung vor der endgültigen Implantation und eine bedarfsgerechte postoperative Steuerung. Bislang liegen keine kontrollierten Studien mit ausreichender Nachbeobachtungszeit (>1 Jahr) beim reinen Rückenschmerz vor. Es gibt allerdings eine Reihe von Beobachtungsstudien beim Postdiskotomieschmerz und bei kombinierten Rücken-Bein-Schmerzen. Insbesondere auf dem Gebiet der Stimulationsverfahren versucht man heute durch neue Stimulationsorte und Stimulationsparameter sowie durch neue Implantate auch den axialen Rückenschmerz besser zu beeinflussen.
