Abstract

Onnisa Nanegrungsunk, MD William F Mieler, MD

Patients with retained intraocular foreign bodies (IOFBs) oftentimes present with a multitude of ocular abnormalities in addition to the IOFB itself. In any open globe injury, one must always be suspicious for the possibility of a retained IOFB. Assessment of the injury needs to be performed in a timely fashion, along with precise localization of the IOFB via a variety of imaging modalities. Then removal of the IOFB and repair of the globe is dependent upon the size, location, and composition of the IOFB(s), along with associated ocular abnormalities. Routes of removal of the IOFB are reviewed, along with the management and prevention of numerous complications including retinal detachment with or without proliferative vitreoretinopathy, hemorrhage, cataract formation, endophthalmitis, metallosis, and other entities. The majority of globes can be salvaged, with varying degrees of visual function. This chapter provides a complete overview of all complexities in the evaluation and management of retained IOFBs.
