Supraclinoid Internal Carotid Artery Aneurysm: Giant Supraclinoid Internal Carotid Artery Aneurysm, Treatment with Extra-Intracranial Bypass and Parent Artery Occlusion

Abstract

A 36-year-old female patient presented with severe headaches and impaired vision. MRI revealed a giant, partially thrombosed aneurysm arising from the left internal carotid artery (ICA). DSA showed the aneurysm neck originating from the supraclinoid segment of the left ICA. The case was discussed with our neurovascular team, with expertise in both surgical and endovascular techniques. After careful consideration was given to the size and location of this complex aneurysm, the parent artery anatomy and the anticipated potential risks of both the natural course and any treatment attempt, the patient was planned to undergo extracranial-intracranial (EC-IC) bypass surgery followed by subsequent endovascular ICA occlusion. Both components of this treatment were well-tolerated, and neither technical nor clinical issues were encountered. A follow-up DSA and MRI 36 months after the treatment demonstrated complete thrombosis of the aneurysm, patency of the EC-IC bypass, and a significant decrease in aneurysm volume. The management of large and giant intracranial aneurysms with a combination of bypass surgery and endovascular parent vessel occlusion is the main topic of this chapter.
