Abstract
Hematopoietic stem cell transplantation is a long and demanding process that can be physically, mentally, and emotionally taxing for patients and their family members. Patients facing transplant need much education about what to expect before, during, and after the transplantation. A comprehensive pretransplant psychosocial assessment is important to identify needs that require intervention early in the course of treatment and as well as close monitoring for emerging psychosocial issues following the transplant process. Emotional distress is common throughout the transplant trajectory. Some patients develop anxiety, depression, and adjustment disorder during HSCT and may benefit from pharmacological and nonpharmacological treatment. One effective nonpharmacological treatment is cognitive behavioral therapy. Delirium in the acute posttransplant phase is not unusual and managing agitation in the context of delirium may require addressing the underlying medical cause and prescription of psychotropic medications for short periods of time. During survivorship, patients adjust to their βnew normal,β and often worry about recurrence of their disease. Major tasks include returning to work/school. Related (adult and pediatric) and unrelated donors experience varied emotions related to donating and benefit from clear information about what to expect. Like patients, they may also benefit from psychosocial support to reduce distress.
