Abstract
The “iHealthHome” is a comprehensive in-home monitoring, collaboration and documentation system designed by professional caregivers for professional caregivers. The system allows older adults to age in place and remain independent and self-sufficient. iHealthHome is a dynamic platform that coordinates interaction and feedback between older adults at home and their care team including professionals, family and service providers. The goal of iHealthHome is to enable older adults to “Live Well at Home” by promoting: self-management of health conditions; remote monitoring for activities in the home; electronic documentation and collaboration among the caregiving team; efficiency and cost reduction of in-home caregiving; and transparency and participation of family members in planning for care and services for older adults remaining at home. The device interface is an easy to use touchscreen monitor that is installed in the home of an older adult. Wireless devices such as motion sensors, blood pressure monitors, glucometers and scales can be added to monitor older adults and their environment. The iHealthHome platform operates alongside professional applications to help caregivers communicate and collaborate.
